The Northern Devon Healthcare NHS Trust uses INRstar’s External Patient Lookup to search for, view and treat anticoagulation patients who are not normally managed by the Trust.

The Northern Devon Healthcare NHS Trust, which runs North Devon District Hospital in Barnstaple, uses this functionality to improve patient safety and reduce adverse events associated with poor access to patients’ anticoagulation records.
LumiraDx Care Solutions developed External Patient Lookup to meet the requirements of healthcare professionals who needed to access the record of a patient not usually managed by their anticoagulation service. The functionality has been developed to ensure special checks are in place to guarantee this would only be for the purpose of direct care, in accordance with Caldicott 2 recommendations. The patient must be able to be uniquely identified by the search criteria and the user must provide a reason for accessing the record, meaning only registered healthcare professionals can use this functionality on a permissions-based log-in.
Jason Pruchniewicz, lead pharmacist for surgery for the Northern Devon Healthcare NHS Trust, helps to run the warfarin clinic at North Devon District Hospital. Jason explains how he is benefiting from the functionality:
“I have used INRstar since 2009, and this new feature is great. All the surgeries in the surrounding area use INRstar, and often patients are coming in and out of hospital quite regularly. We use External Patient Lookup to look up a patient once they are admitted, and we use it to identify the dose of warfarin they are on. Patients often don’t have their ‘yellow book’ with them, so we can check we have the patient on the same dose and the prescriber can then update the patient’s information. This means that when the patient is discharged, and when they return to their surgery, the dosage information has been updated. This is really useful, and helps to ensure a safe level of dosing.”
“Another area in which I find the system really useful is when I carry out pre-op assessments. When patients come in for surgery, I use External Patient Lookup to get their details. So if they are stopping using warfarin during their stay, I can update their notes and this will be read by everyone who needs to know. I use the system all day, every day and it’s really helpful to me. I also use the solution for holidaymakers visiting the area. Again, if they are on INRstar, I can find out all the background information required to give them safer care.”
